Criteria

IMPORTANT: Carefully read and follow all instructions to ensure your application is complete. Incomplete applications will result delays or can result in your application being rejected. Taking the time to double-check everything helps avoid unnecessary setbacks and ensures your application is fully considered.  

If completing the web application (green “Apply Now” button):

• Complete all sections. Incomplete applications will NOT be considered.
• Enter your email address and submit.
• Go to your EMAIL. You will receive an email from ADOBE.  Open it and click on “confirm your email address”.

If completing application by hand:

• Download & print application (gray “application” button).
• Complete all sections. Incomplete applications will NOT be considered.
• Upload application to this website (See “upload documents” button below).

Release of Information (ROI):

• Download & print the ROI (gray “ROI” button).
• Complete ROI, sign & date. Do not sign and date section to  withdraw consent.
• All ROI are to be PRINTED, COMPLETED AND SIGNED IN WET INK. Electronic signatures will NOT be accepted for these forms. 
• Upload ROI to this website (See “upload documents” button below).
